Case 1: Short Scar Periareolar Inferior Pedicle Reduction (SPAIR) Mammaplasty
Several techniques for breast reduction with a reduced scar burden have been described over the past 30 years. Obtaining consistent aesthetic results that maintain an aesthetic shape over time can be challenging. The SPAIR procedure was conceived and refined with this in mind. This procedure relies on an inferior pedicle used in conjunction with a combined periareolar and vertical (circumvertical) skin pattern. The inframammary fold is preserved, which contributes tremendously to stabilizing the postoperative shape of the breast. The technique is easily applied in breasts up to and exceeding 1000 grams per side, leaving the patient with only a periareolar and J-shaped vertical scar. This video describes in detail the marking principles and surgical technique of the basic SPAIR procedure.

Case 2: Short Scar Periareolar Inferior Pedicle Reduction (SPAIR) Mammaplasty With Internal Shaping
Many patients present for breast reduction and mastopexy with a hollowed out upper pole of the breast. Traditional techniques of breast reduction that rely only on a skin brassiere to reshape the breast often fail to adequately restore fullness to the upper pole in these patients. This video highlights the internal breast-shaping concepts used in conjunction with the basic SPAIR breast reduction procedure that result in restoring fullness and shape to the upper pole, thereby creating superior aesthetic results in this challenging group of patients.

Case 3: Second Stage Implant-Based Breast Reconstruction
This video highlights the surgical steps required to obtain superior results in two-stage immediate nipple-sparing implant-based breast reconstruction. Important concepts regarding implant selection, ADM use, pocket management, and fat grafting are described in detail. The rationale for the two-stage tissue expander to implant-based approach is presented, leaving the viewer with a better understanding of why this sequencing can lead to consistent, predictable, and highly superior aesthetic results.

Case 4: Advanced Concepts in Inverted-T Inferior Pedicle Breast Reduction
The inverted-T inferior pedicle breast reduction is considered the gold standard against which all other techniques are compared. However, common complications related to bottoming out of the breast with loss of contour and shape are observed with this technique. This video highlights new anatomic principles related to the inframammary fold and the internal breast septum that are crucial in providing consistently safe results. In particular, simple preoperative marking principles combined with advanced shaping maneuvers related to stabilization of the inframammary fold and suture plication of the internal breast contours are presented, resulting in consistently superior aesthetic outcomes.
Use of the principles described in this video will transform what has previously been a tried-and-true technique for breast reduction into an effective means of meeting the increasingly demanding aesthetic requirements of this frequently challenging group of patients.
